Enlightenment Transitional Living provides services to eligible members enrolled in Tennessee’s Employment and Community First CHOICES (ECF CHOICES) and CHOICES programs.

Not currently enrolled? Use the links below to learn more, check program requirements, or begin the referral process through the Tennessee Department of Disability and Aging or TennCare.

People who are not currently enrolled in TennCare can contact their local Area Agency on Aging and Disability or call 1-866-836-6678. Current TennCare members should contact the health plan listed on their TennCare card.

Under certain Tennessee Medicaid programs, eligible family members may be able to become paid caregivers for their loved one.

Reliable transportation to approved appointments, work, community activities, shopping, and other destinations.

Supported Living
Individualized support that helps adults live safely and successfully while building independence and maintaining meaningful routines.

Semi-Independent Living
Support for adults who need some assistance while developing the skills and confidence to live more independently.
